・Foreign people living in Japan with status of residence over 3 months.

You need to check the website of your municipalities.

Who can Apply

Head of Household

✏️If the head of household is not you, you need to ask the head of household to apply for all of your household.

✏️If you have registered your residential card with your host's address, you can ask your host to apply for you.

Deadline of Application

3 months after your municipal ward office start accepting the paper style applications
